The patient referral guide for practices describes in detail the various methods of submitting patient contact information to the PREPARE 4 CGM study team. Practices can use the example patient names spreadsheet to track and send patient contact information to the study. As a reminder, please do not send patient contact information until your practice’s BAA or DTUA has been fully executed. 

Patient recruitment flyers in English and Spanish are available. To request printed copies of this resource, please email prepare4cgm@cuanschutz.edu and include the number requested in each language as well as the mailing address for the practice. 

• Track information about patients invited to participate in study: name and contact information
• With the support of the CGM project team, report A1c and BMI for patients participating in the study.  Practice may elect to report this clinical data or have the CGM project team perform chart audits.
• Complete baseline, 6- and 12-month surveys (e.g., aggregate demographics of patient population, implementation of various CGM activities, number of CGM prescriptions)
  • The guide for tracking CGM prescriptions includes ideas and guidance for tracking the number of CGM prescriptions in your practice. The sample patient registry is one option for tracking the number of prescriptions.
• A few practices will be selected for site visits and brief interviews of three practice members regarding the project. Practice members who take part in interviews will receive a $50 gift card for participation.

The AAFP CGM toolkit includes training, tools, and information that will be beneficial to clinicians as well as staff, such as nurses, medical assistants, practice managers, care managers, billing specialists, and pharmacists. It is intended for at least one person in the practice to take the course (4.25 accredited hours of continuing education) and then to bring to the tools to the rest of the practice as a practice-wide implementation project. The updated version now includes updated evidence, updated (and expanded) insurance eligibility, and updated resources and is available through February 2, 2026.
