provided the Department of Health Care Policy and Financing (HCPF) funding to support, improve, and expand integrated behavioral health services in primary care settings in Â鶹´«Ã½¸ßÇå. As part of this program, the Practice Innovation Program provides:

• practice facilitation support to awarded practices through our CHES Practice Transformation Organization (PTO) partners
• management and quality assurance of practice transformation
• subject matter experts to support the behavioral health implementation goals of the awarded practices
