DO NOT CLOSE YOUR BANK ACCOUNT until your last paycheck has cleared. End-of-June paychecks will be direct deposited on the last business day of the month (June 28, 2019). If you have additional money due after your last regular paycheck, e.g. internal moonlighting or additional payments, this will also be direct deposited, so it is recommended that you do not close your bank account until all expected payments have cleared. To avoid delay in receiving your W2, keep your address updated with Employee Services (303-860-4200).​ 

All insurance benefits end at midnight (unless completing a shift) on your final day in the training program (if exiting in June). You have no “grace period.”  If you are not sure of your correct ending date, contact your residency/fellowship program director to verify.

CU GME Residents/Fellows may choose COBRA continuation coverage for the medical, dental and vision plan(s) in which they were enrolled immediately prior to termination of active coverage. View CU GME Health, Dental or Vision Care COBRA Continuation of Coverage Information and Rates (opens pdf).

CU Employee Services will notify ASI COBRA, the claims administrator, of your termination of coverage. 

ASI COBRA will mail the Notification of Right to Elect Continuation of Group Health Benefits/COBRA to your home address. This will include Election/Enrollment Form and premium rate information. You may also use this Notification as proof of termination of your CU Health/Dental/Vision Plan coverage. 

For known June and July exits, ASI COBRA will mail this information to your home address in mid-July. If you do not receive it or misplace it, call  ASI COBRA, 1-877-388-8331, for instructions on how to obtain a copy. Your individualized copy of the above information must be obtained directly from ASI COBRA.

In addition to the CU Health Benefits Plan COBRA continuation of coverage, other health coverage alternatives may be available to you, including but not limited to an individual interim insurance plan, coverage through the Health Insurance Marketplace () or a special enrollment opportunity for another group health plan for which you are eligible (such as a plan sponsored by your spouse’s employer), even if the plan generally does not accept late enrollees, if you request enrollment within 30 days. 

Group long-term disability coverage ends. Guaranteed-issue conversion to individual disability policy is available. See CU GME Disability Conversion Coverage​ information.

Life insurance coverage ends. You may have options for conversion or portability of your current group life insurance within 31 days of exit through Standard Insurance Company. Further information is posted on the GME Benefits website on the "Life Insurance" tab. Contact Standard Insurance Company Continued Benefits at 1-800-378-4668 with questions.

Immunization/TB test records are not maintained within GME and are not accessible at or through the GME Office. You may email PROHealth, the designated provider for CU GME’s immunization program, at PROHealth@dhha.org to request a copy of your immunization documentation. Phone requests are not permissible. Your email will serve as your consent for release of records. Records will be provided only to you (no third parties) via email. You should keep a copy of these records and can then provide directly to requesting entities.

To request proof of malpractice coverage (for licensure, etc.), call 303-724-6031 or email gme@ucdenver.edu. Proof of coverage will be provided to the resident only (no third parties, unless specific permission and instructions are given by the resident to do so) via mail, fax, email or picking up by the resident from the GME office. Please specify in your request how you want to receive. When applying for a COLORADO license, GME will send directly to the Â鶹´«Ã½¸ßÇå Board of Medical Examiners. If you would like a copy of what is sent to the Board, please provide an email address in your request of where to send a PDF copy of your coverage form letter. Request for claims history must come directly from and is provided directly to the requesting entity. GME does not release your claims history directly to you. 

Requests for verification of training used to obtain privileges with hospitals, insurance plans, etc., should be sent directly to the office of your training program director.  Give your director’s correct address, including campus box number, when completing forms for privileges.

GME Parking privileges end on the last day of your training. Parking access through GME will be turned off.  If you have a VA parking key card, please return the tag to the GME Office at the Anschutz Medical Campus (Fitzsimons Building, 4th Floor, Room N4223).  Office hours are Monday through Friday, 8:00 am to 4:30 pm.  Office phone number is 303-724-6031. Information from Parking and Transportation on unpaid citations will be mailed directly to your home address. Please call the Parking Office, 303-724-2555, for any further information.

GME PAGER use ends. Turn in your pager to your program coordinator along with your CU GME Clearance Form (opens pdf). Your pager will be turned off.

CU Denver alumni have Email For Life!  As CU Anschutz alumni, you may continue to use your ucdenver.edu email account. If you are currently using your email, you do not need “claim” the account again. If you have not been using your ucdenver.edu email account, claiming your ucdenver.edu email address will ensure that you are up-to-date on all happenings at CU Anschutz, and that your CU Anschutz friends, colleagues, faculty, and staff will be able to communicate with you. Resident will need to complete a CU GME Clearance Form (opens pdf). You are required to obtain a sign-off from the health information management (medical records) department of each facility (except UCH) where you saw patients during this academic year. On your last day, your final stop will be with your program coordinator, where you will return any university-issued property, e.g. pager, keys, lab coats, CU ID badge, etc., and submit your CU GME Clearance Form so your Program Coordinator can complete your check out. 

IF YOU ARE BECOMING A CU FACULTY MEMBER:

GME Insurance Benefits Ends:

• You must enroll in a faculty health insurance plan in order to receive benefits. Contact your new department/division administrator now to find out how to enroll in a faculty benefits plan. If you need to purchase interim coverage, you may want to review the above information on health, dental or vision care continuation of coverage.

GME Parking Privileges End on the Last Day of Training:

• Contact the parking office at 303-724-2555 to arrange for new parking privileges.

CU E-mail Account:

• Contact your hiring department and request that they transfer your job record in HCM so that your account does not get disabled.

GME Pager Use Ends:

• Your pager will be turned off and must be turned in to your coordinator.  If it is decided you are to retain a specific pager number, contact your department/division administrator to arrange.  (The pager funding must be transferred to a different source. The GME source will be discontinued as of your last day of training.)
