Clinical Care for Adult Patients
The Adult Clinic has 9 providers (6 Physicians and 3 Advanced Practice Providers) who provide care for approximately 3,500 patients per year. Each patient has the opportunity to receive diabetes education at any clinic visit. The education is provided by nurses, dietitians and social workers (all are Certified Diabetes Educators). The diabetes educators provide education on self-management of diabetes for patients on multiple daily injections or insulin pumps. The education includes instruction on topics such as carbohydrate counting, weight and life-style management and psych-social counseling.

Emerging diabetes technologies are discussed with patients at each clinic visit. Patients are advised on recent advances in technology and which technology might be best for them on an ongoing basis. Patients who choose to start an insulin pump will be trained on the features and use of the system (pump and continuous glucose monitor) in a group class on the 1st or 3rd Wednesday of each month by appointment only. Patients will be followed remotely (tele-visit) by 1-2 weeks after the pump initiation and then come to the clinic for a follow up appointment 3-4 weeks after the pump initiation.
Patients who choose to start a Continuous Glucose Monitor (CGM) can come to the clinic for training and can call 303-724-6755 to set up an appointment. Patients will be trained on the features of the device and how to interpret the CGM data provided by the device to manage their diabetes. Patients are taught how to download their data and its importance.
Macro-vascular monitoring is provided on a routine basis to prevent long-term complications. Patients will have an EKG and Peripheral Vascular every 1-3 years as determined by their provider.
The Adult Diabetes Division offers special care for women with diabetes (pre-existing or gestational) to actively manage glucose levels throughout pregnancy. Women progress from insulin sensitivity in the first trimester to progressive insulin resistance in the second and third trimesters to insulin sensitivity once again in the early post-partum period. This includes the care of women on multiple daily injections of insulin, continuous subcutaneous insulin infusion therapy, and/or continuous glucose monitoring therapy. Adult clinic providers proactively monitor for progression of diabetic complications that may occur during pregnancy, such as eye and kidney disease and hypertension. Additional services include:
